Event: 5K Fun Run and 1 Mile Walk

Participants CAN register the day of event.

Participants should be present at 7:45 a.m. at the Fire Station, 813 Main Street.  Medals awarded to   1^st- 3^rd place in the 5K Run (male & female awarded separately) in each category.  Medals will be awarded to the first 3 finishers in the 1 Mile Walk.

WAIVER & INDEMNITY AGREEMENT
(Please read carefully before signing)

By my signature below, and in consideration of the participation in the 2011 Boyden Bash Celebration, I do waive and release and do absolve the City of  Boyden, the Boyden Park Board, the Boyden Community Club and all individual members thereof, all employees or agents of the City of Boyden, the Park Board and the Boyden Community Club, and any other individual responsible for directing, conducting or otherwise involved in the 2011 Boyden Bash Celebration, from any liability for any cause of action, claim or demand of any nature which might arise in conjunction with the participation in the 2011 Boyden Bash Celebration. It is my/our intention that this waiver apply as well to my heirs, assigns, executors and administrators.Furthermore, I/we do here agree to indemnify and hold harmless the same persons and entities from liability under any cause of action, claim or demand including, but not limited to loss of consortium or loss of support, made by my spouse, children or any other person as a result of, arising out of, or in conjunction with any injury resulting from or in connection with the 2011 Boyden Bash Celebration. This agreement shall also be binding upon my heirs, assigns, executors and administrators.

It is further understood by putting my name in the following form and accepting one or the other option, it will constitute my formal written signature, whether I am an adult, or yet to be of age, the full understanding of this waiver to participate in Boyden Bash events.
